U S is caught in a typical situation - NICE to have OR NEED to have.
On one hand it equally important to ensure that the entire population is covered of their health care needs. While on the other hand the immediate question Can we afford?
So it is a debate of "AFFORDABILITY" v/s "COMPULSIONS"
